Name changes:
Name changes must be submitted in writing, and should include:
- Full original name and address;
- Certification number or membership number;
- New full legal name;
- Copy of marriage certificate or court order;
- Copy of state issued photo id with either old or new name; and
- Verification of your current contact information: home address, email, and phone.
