Name changes:
Name changes must be submitted in writing, and should include:

  • Full original name and address;
  • Certification number or membership number;
  • New full legal name;
  • Copy of marriage certificate or court order;
  • Copy of state issued photo id with either old or new name; and
  • Verification of your current contact information: home address, email, and phone.
